We want to give you the best experience at TMW by bringing you quality care with a personal touch. Before we can do that we do ask that you print and fill out this form that will be e mailed to you personally. The massage intake form is used so your therapist can customize the massage and be sure they have an understanding of your history and what you are looking to achieve from your session. Please bring on the day of your scheduled appointment or feel free to fill it out when you arrive.
